Epidemiological and preventive aspects of Hepatitis C virus infection in Ukraine

Viral hepatitis is a common and dangerous infectious disease caused by various hepatitis viruses, which primarily affect human liver cells. Hepatitis C is a liver disease caused by the hepatitis C virus (HCV): the virus can cause both acute and chronic hepatitis, ranging in severity from a mild illness lasting a few weeks to a serious, lifelong illness. Analysis of materials obtained in of the official registration of cases of the Ministry of Health of Ukraine, a total of 44359 cases of HCV infection were registered in country for 7 years (2013 - 2019), including 3826 cases of Acute hepatitis C - (8 , 44 ± 0.48)% and 41483 Chronic hepatitis C - (91.56 ± 0.48)%

ORGANIZATIONAL PRINCIPLES OF INTERNATIONAL MEDICAL TOURISM CLUSTER MANAGEMENT

Objective: This article aims to present an economic rationale for a model of managing a medical tourism cluster, shedding light on the organizational principles and key components of this vital industry. Methods: The research employs a systematic approach to delineate the components of the medical tourism cluster, categorizing it into diagnostic and therapeutic, wellness, and beauty tourism. Results: Developed the map of cluster medical tourism and identifies its main components. Specific interpretations and entity definitions types of medical tourism. Identifies the main international and national associations, which coordinate the activities in the field of medical tourism. Justified model of the cluster management medical tourism, developed the organizational structure of a public organization that performs a coordinating influence on the activity of the cluster of medical tourism. Conclusion: In conclusion, this research provides valuable insights into the organizational and economic aspects of managing a medical tourism cluster. By delineating the structure, stakeholders, and coordination mechanisms, it offers a robust framework for enhancing the competitiveness of the medical tourism industry.
Portopulmonary Hypertension (Literature Review)

Portopulmonary hypertension is a form of pulmonary arterial hypertension that develops as a complication of portal hypertension. The specific gravity of liver cirrhosis as a cause of portal hypertension is about 75%, which means that a significant proportion of patients with portopulmonary hypertension have liver cirrhosis, therefore, these patients are potentially candidates for liver transplantation. Although this complication is not common, it is extremely important to distinguish portopulmonary hypertension from other causes of pulmonary arterial hypertension, since an increase in mean pulmonary arterial pressure > 35 mmHg is associated with a 50-100% mortality rate during waiting and after liver transplantation. There are no clear recommendations for the treatment of this complication in patients with portal hypertension, since there are not enough clinical trials in this group of patients. A significant number of obstacles can limit the adequate treatment of patients with portopulmonary hypertension and explain the lower survival rate of this group of patients compared with other types of pulmonary arterial hypertension. Until recently, only one randomized controlled trial included patients with portopulmonary hypertension, and most of the treatment data came from relatively small observational studies. Currently, the treatment of portopulmonary hypertension includes therapy specific for pulmonary arterial hypertension regardless of its cause, and in some cases such therapy is necessary to facilitate successful liver transplantation.
Possible strategies for the implementation of WHO program on the elimination of hepatitis C in Ukraine by 2030.

The article describes the model of natural history of hepatitis C virus the use of which makes it possible to assess the dynamics of the infected population from 2015 to 2030 in natural history and under the influence of the strategies of antiviral therapy, namely, the changes in the number of infected populations, the progression of the diseases, patients’ proportion with different stages of fibrosis and also the mortality which is associated with final stages of liver disease. In addition, there are estimated data on the number of the patients that are subject to annual diagnosing for treatment in accordance with specific therapy strategies which is necessary to achieve the results provided by the WHO program for the elimination of hepatitis C virus in Ukraine by 2030.
