The likelihood of such prompt life-threatening septic complications, as thrombophlebitis, pneumonia, pleurisy, etc, may be underestimated due to the small size of the local inflammation of the lip. We present a 21-year-old Caucasian female with a Staphylococcus epidermidis infection of the lower lip (furuncle) complicated by lip abscess, suppurative cheilitis glandularis, lip necrosis, septic bilateral multilobar necrotizing (destructive) pneumonia, pleurisy (pleuritis), and pulmonary insufficiency of the first degree (type I). Clinical photos of the report demonstrate all stages of the course of inflammation and treatment of the severe lower lip infection from the moment of admission. Preoperative condition, the period of formation and demarcation of necrosis, cleaning of the wound from purulent content, demonstration of fibrinous exudate (also known as fibrinous plaques), the appearance of granulations, and visualization of lip scars as of the day of discharge from the hospital (day 26) are presented. The article also includes a chest X-ray at the time of admission, on the second day, and on the seventh day of hospital stay. The report is enriched as well by three-dimensional multi-slice computed tomography (3D MSCT) images on the eighth, eighteenth, and fortieth days from the moment of admission to our hospital. 3D MSCT of the lungs is showing the areas of pulmonary tissue necrosis and the healing phase. The article contains two videos (cine images) with a duration of 21 seconds and 2 minutes 11 seconds, which demonstrate chest MSCT on the hospital day 8 and 18. The patient was discharged on the 27th day of admission. According to our English-language literature search, it's a first ever reported case of the lower lip furuncle which led such a unique combination of severe local and septic complications documented at all stages of treatment from both aspects clinical and radiological. An update of the classification of uncomplicated and complicated forms of development of furuncles (i.e., boils) and carbuncles is done and presented. More than 37 literary sources were analyzed, and their key aspects complemented our report. Articles with necrotizing fasciitis of the lips were also analyzed. Інфекція нижньої губи може проявлятися по-різному. Вірогідність виникнення таких швидко загрозливих для життя септичних ускладнень, як тромбофлебіт, пневмонія, плеврит тощо, може бути недооцінена через невеликі розміри локального запалення губи. Представляємо вашій увазі 21-річну жінку європеоїдної раси з інфекцією нижньої губи (фурункул), спричиненою епідермальним стафілококом (Staphylococcus epidermidis), ускладненою абсцесом губи, гнійним гландулярним хейлітом, некрозом губи, септичною двобічною полісегментарною некротичною (деструктивною) пневмонією, плевритом і легеневою недостатністю I ступеня. Клінічні фотографії звіту демонструють усі етапи перебігу запалення та лікування важкої інфекції нижньої губи з моменту надходження пацієнтки. Представлено передопераційний стан, період утворення і відмежування некрозу, очищення рани від гнійного вмісту, виявлення нальоту фібрину, появу грануляцій, вигляд рубців на губі на день виписки зі стаціонару (26-й день). У статтю включено також рентгенографію органів грудної клітки під час госпіталізації, на другу добу та на сьому добу перебування в лікарні. Також звіт доповнено зображеннями тривимірної мультиспіральної комп’ютерної томографії (3D МСКТ) на восьму, вісімнадцяту та сорокову добу з моменту надходження до нашої лікарні. 3D МСКТ легенів показує ділянки некрозу легеневої тканини та фазу загоєння. У статті подано два відеоматеріали (кінокадри) тривалістю 21 секунда та 2 хвилини 11 секунд, які демонструють МСКТ грудної клітки на 8 та 18 день стаціонару. Пацієнтка виписана на 27 день госпіталізації. Згідно з нашим пошуком англомовної літератури, це перший в історії випадок фурункула нижньої губи, який спричинив таке унікальне поєднання важких місцевих і септичних ускладнень і які були задокументовані на всіх етапах лікування з обох аспектів, клінічного та радіологічного. Зроблено та подано оновлення класифікації неускладнених та ускладнених форм розвитку фурункулів (тобто фурункулів) і карбункулів. Було проаналізовано більш ніж 37 літературних джерел, основні аспекти яких доповнили наш звіт. Статті з некротизуючим фасціїтом губ також проаналізовані.Item Clash of Videos in the Cutting-Edge Medical Publications(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2017-12-28) Tymofieiev, OleksiiItem Condition of the Teeth in Fracture Gap of the Mandible(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2017-02-18) Tymofieiev, Oleksii; Fesenko, Ievgen; Tymofieiev, OlexanderPurpose: The aim of the present study was to determine condition of the teeth, which are located in the fracture gap of different parts of mandible and substantiate an indications for their removing or retaining. Material and Methods: 114 patients with 186 fractures were involved in this research. Results: 50% or more cases of post-traumatic inflammatory complications were observed in case of contact failure of the teeth roots, that are in a gap of fracture, with the periodontium of these teeth al 1/2 or more of their length. Saving teeth in fracture gap, which have failure of contact with the surrounding periodontium 1/2 and 3/4 the length of the root causes high risk of inflammatory complications in bone and surrounding soft tissues. An institutional repository is one of four types of repositories. The purposes of this article are (1) to highlight the most important points of the Regulation on the Institutional Repository of the Kyiv Medical University (IRKMU) and (2) to explain the advantages for authors and peer-reviewed journals of colonization of the repository of such type. Also, this paper highlights the experience of the Department of Oral and Maxillofacial Surgery and the Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology (JDTOMP) in depositing peer-reviewed articles in this repository. The Regulation on the IRKMU was put into effect by Order No. 29 of January 26, 2024. In the same year, all structural units of the university, including dental departments, joined the process of depositing their scientific and clinical works. The IRKMU was created using the open-source software DSpace, which supports the Open Access Initiative Protocol for Metadata Harvesting (OAI-PMH) data exchange protocol, allowing the repository to be integrated into the international registers of the Registry of Open Access Repositories (ROAR), Directory of Open Access Repositories (OpenDOAR), and other search engines. The article is reinforced with screenshots of the stages of depositing data of scientific works. Carlile Butterfield and colleagues (2022) emphasized, that despite all benefits, however, their research indicates that it can be difficult to encourage all faculties to upload the research reports onto the institutional repository. Moreover, the authors propose a potential route to maximize the number of publications that are uploaded to a university institutional repository. They utilize student workers and the resources of the institutional repository manager to get materials uploaded. The experience of our department consists in attracting a competent editor from the editorial staff of the JDTOMP with appropriate payment for services. In sum, colonizing this new institutional repository as a new database, authors and peer-reviewed journals can and will increase the visibility of their publications. Thus, it can bear fruit in terms of increasing the citation of articles and the journals as well.Item Editorial: The First Impact Factor of the Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2024-08-31) Tymofieiev, Oleksii; Fesenko, IevgenThe journal impact factor (JIF) is a measure that provides a ratio of citations to a journal in a given year to the citable items in the prior two years. The JIF has become a symbol and measure of a journal's prestige in the modern world of peer-reviewed publications. The difficulty and importance for a peer-reviewed journal to obtain an impact factor has been highlighted in numerous medical journals. Irving H. Sher and Eugene Garfield were the first to create the concept of the impact factor of a peer-reviewed journal. According to Eugene Garfield (2006), the JIF is based on two elements: the numerator, which is the number of citations in the current year to items published in the previous two years, and the denominator, which is the number of substantive articles and reviews published in the same two years. Scientists around the world may notice that as of 2024, the JIF is calculated not only by Clarivate, but also by exaly. Clarivate is known for being the company that calculates the JIF, using data from its Web of Science product family, that also includes services/applications such as Publons, etc. In this editorial, we will explain in more detail why the JIF is important both for the peer-reviewed journals published in Ukraine and for Ukrainian authors. Also, let's also explain how both the impact factor calculated by Web of Science and the impact factor calculated by exaly can be useful. The article presents the experience of the Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology in assigning it an impact factor calculated by the exaly. The advantages for Ukrainian authors of publishing articles in the journals with an impact factor are presented. According to the Order of the Ministry of Healthcare of Ukraine of February 22, 2019, No. 446, publication of an article or review in a journal with an impact factor allows an oral and maxillofacial surgeon to be awarded 20 or 30 continuing professional development (CPD) points. The CPD points obtained are necessary for doctors to the annual review of their personal educational portfolio with CPD points, certification, confirmation or obtaining a new doctor’s category. In summary, the peer-reviewed journal that has received an impact factor automatically becomes a kind of provider of CPD activities for healthcare professionals, for which CPD points are awarded.Item Editorial: The V. And such a requirement is the inclusion of the journal and the deposit of its articles in the V. I. Vernadskyi National Library of Ukraine (VNLU). So, in this editorial we will highlight the experience of the Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology of including it in the VNLU. The VNLU is a leading cultural, educational, scientific, and informational state institution that performs the functions of a research, methodological, and coordination center on issues of book studies, library science, bibliography, and document studies, and participates in the development of state policy in the field of library science and its implementation. All details about the Library's day-to-day operations and functioning can be found in its Bylaws, which is publicly available. What is important to know is that the VNLU has the right to receive a mandatory free copy of documents in accordance with the legislation. The deposit of peer-reviewed journal articles in the VNLU is one of the extremely important stages in the evolution of each peer-reviewed journal originating from Ukraine. And the deposit process itself is quite simple and does not require a significant amount of editorial and publisher resources. In summary, the VNLU is another highly respected state database that contains our journal and its articles, along with the National Repository of Academic Texts and ResearchGate.Item Evolving: Becoming a Printable Digital-only Journal from January 2022(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2021-12-31) Tymofieiev, Oleksii; Fesenko, Ievgen; Kilipiris, EvangelosItem Facial Nerve Neuropathy Caused by its Stretching(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2017-06-30) Tymofieiev, Oleksii; Kryvosheieva, Аnna; Beridze, BekaPurpose: Study the dynamics of changes in electrophysiological parameters of the facial nerve in cases after operative interventions on the parotid and submandibular glands, as well as determine the possibility of using electrical stimulation of the nerve on the hardware-software complex “DIN-1” for the treatment of neuropathy caused by its stretching. Patients and Methods: 31 patients were examined after surgery on the parotid and submandibular glands. Results: Based on the performed patient examinations after parotidectomy and extirpation of the submandibular glands, it was established that in the postoperative period they may have neuropathy of the facial nerve, the cause of which is the stretching of the nerve. Clinical improvement and elimination of symptoms were correlated with normalization of electrophysiological indices of the state of affected branches of the facial nerve. Conclusions: Due to the use of the hardware-software complex “DIN-1” for diagnosis and treatment of the facial nerve neuropathy caused by its stretching, we managed not only to identify and prove the existence of this disease, but also to shorten the terms of rehabilitation of patients 4 times.Item Features of Diagnostics, Clinical Course and Treatment of the Branchial Cleft Cysts(Journal of Diagnostics and Treatment of Oral and Maxillofacial Pathology, 2017-02-18) Tymofieiev, Oleksii; Fesenko, Ievgen; Cherniak, Olha; Zaritska, ValentynaPurpose: The aim of the present study was to determine the features of diagnostics, clinical course and treatment of the branchial cleft cysts. Patients and Methods: The study composed of the branchial cleft cysts investigation and their complications in patients of different age groups, methods of diagnostics, anatomical features, surgical stages and pathomorphological study. Results: Diagnostic value of sonography, MDCT and MRI, pathomorphological study in verification of branchial cleft cysts and their complications have been proved. Surgical treatment technique is presented.
